Advanced Directives State Description

The State of Oklahoma addresses its Advanced Directive Provisions under the Health & Safety Code, Title 63, Chapter 60.

This provision states that an individual has the right to an Advanced Directive.

(1) “Directive” means an instruction made under Section 3101.4 to administer, withhold, or withdraw life-sustaining treatment in the event of a terminal or irreversible condition.
(2) “Qualified patient” means a patient with a terminal or irreversible condition that has been diagnosed and certified in writing by the attending physician.

FSC Physicians

Your physician may be a partner with ownership of Foundation Surgery Center of Oklahoma. A copy of the list of physician partners is available upon request and is posted at all centers.

The Physicians and Allied Health Professionals practicing at Foundation Surgery Center are licensed and /or credentialed to practice in this facility. The physicians and AHPs provide medical services at Foundation Surgery Center, but they are not agents or employees of Foundation Surgery Center.

It is the policy of Foundation Surgery Center to not follow any Pre-determined Advanced Directives.

All necessary action will be taken to preserve life in an emergency situation, and provide the patient with their rights.
